To set up an account you need the following:

Primary Account Holder:  Photo ID with current address, or, photo ID with a current utility bill, rent receipt, or other business mail that shows current address.
Spouse, Partner, Parent, or Other Adults in Household:  Photo ID with same address as Primary Account Holder, or photo ID with a current utility bill, rent receipt, or other business addressed to them that shows address of Primary Account Holder.

Client’s Children under the age of 18:   Birth certificate.
Client’s Children over the age of 18:  Photo ID showing parent’s address, or, a piece of business mail addressed to child at the parent’s address.
Other Children Living at Primary Account Holder’s Address:   Court ordered documents or tax return documents indicating that the Primary Account Holder is the legal guardian.

For all subsequent visits, the Primary Account Holder must produce a photo ID with their current address.
